TYPES OF DOCUMENTS:
These are some of the types of documents, some legal and some not, that you might use to make your life and the lives of those who will take care of you and your assets easier. Of course, your attorney may have other ideas.
- General durable power of attorney
- Durable power of attorney with authority to make gifts
- Power of attorney with respect to health care and certain other matters
- Living will declaration
- Appointment of health care representative
- Last will and testament
- Revocable trust (one or more)
- Title your car and motor cycle into the trust name
- Title your house into the trust name
- Move your equity accounts into the trust name
- Make a list of assets not in the trust and answer why they are not in the trust
- Special needs trust
- Authorization for release of protected health information
- Assignment of personal and household effects
GATHER:
Gather these items. It will make it easier for your trustee/executor.
- One copy of each of your checking account statements.
- One copy of each of your saving account statements
- One copy of each of your stock broker accounts
- Documents for all outstanding loans
- Deed to your house
- Title to your cars
- Registrations to your cars
- Login and passwords to your internet accounts – although you should use a password manager like LastPass or 1Password
- On line banking
- Etc.
- Notes to the trustee:
- Distribution of items such as furniture, jewelry, etc.
- IRA/Roth
- 401k, 403b
- Get a free credit report from one of the 3 agencies.
- Safety deposit box location, number, and key (Make sure you’re not the only one who has access to the box)
- Copies of the items listed under TYPES OF DOCUMENTS
- Beneficiary forms
- When all the above is done, scan these items into individual files and put them into the cloud, on a cd, or someplace else outside your house. Give someone access to it.
